I just got my new copy of the game today, put it in my new PS4, and get this:

The framerate is also very slow, and I can barely make it past the opening logos. Anyone have an idea of what's going on here?

I don't own a PS4 so my knowledge of it is limited, but given there is only so much you can do with a console to fix this sort of thing, my suggestion would be reinstall the game. I also read that playing the game before it is finished downloading (some games allow for this) can cause problems.

If you haven't tried any other games lately, it might be worth briefly trying to eliminate the possibility of something critical happening with the PS4 itself.

Update: Tried reinstalling the game, same issue. I don't think there's anything wrong with the system, as other games I have downloaded work fine, although because they are downloaded and Nier is a disc, it is possible there's something wrong with the disc drive. I've looked up the error code, and the instructions for it say that if all else fails I need to initialize the system. I'm gonna go ahead and do that now. Wish me luck.

Update 2: finished initializing, issue still persists with Nier. Downloaded games and applications now have the issue as well.
The games and apps I have tested that dont work are: nier automata, Let it die, and Netflix.
The ones that DO work are: Blacklight Retribution, Amazon Video, and Plex.
Does anyone know if these software have anything in common?

That does look like an issue with the graphics card. Don't know anything about PS4s but if it is in fact a hardware issue I doubt you can do much besides sending it to get it fixed.

I agree with everyone else who says it's most likely a graphics card issue. The only other thing I can think of is messing around with the video output settings and hope for the best.

I would say make sure you have the latest firmware for the console, and software updates as others mentioned.

Also, are you using a HDMI splitter / switch (based on your high quality screenshot)? If you are, remove that from the equation, and connect the PS4 directly to your TV and see if the issue still exists. Also, try switching out HDMI cables (I've seen these randomly die after 5+ years in service).

I wouldn't be surprised if your PS4 is stuffed, I'm on my 3rd one now because of hardware issues.

As people thought, it was a firmware issue. nothing to be done but get it replaced.
